Ron Wolf was the one who recommended McCoy and Dean fell in love him and hired him, cancelling an interview with Bruce Arians and not bothering to interview Andy Reid. My understanding is that Dean let TT and John hire the next coach and that was Lynn. After that disaster, Dean was much more involved in the hiring of Staley. It was a team cluster-f*** for all of the coaches hired.

Good coach, but I think the stresses that come with all of the hours put into being a head coach are too much for his heart-related issues.

Spanos: Why should we hire you as GM?
GM prospect: I'll keep Staley.
Spanos: You're hired!!
GM prospect thinking to self: damn I've got job security for the next decade playoffs or not.

As for a GM bringing in the head coach - well for Jim Harbaugh - it would work the other way around
